The Average Med Spa Is Empty More Than Half the Day

The Average Med Spa Is Empty More Than Half the Day

Someone cancels at 11am. The 2pm slot just sits there, and by the time anyone thinks about it the day is over. Most clinics treat that as bad luck. It is the cheapest revenue in the building, and there is a simple system for catching it.

Someone cancels at 11am. Your 2pm is now a hole.

You mean to do something about it. Then you are back in a room with a patient, and by the time you look up it is 4pm and the day is gone.

That slot was already paid for. The rent, the provider's hours, the ads that filled the rest of the day. This is the cheapest revenue in your clinic and almost nobody has a system for it.

1. The Number Almost Nobody Tracks

Zenoti's 2026 Beauty and Wellness Benchmark Report, built on 2025 data from North American businesses on their platform, found something worth staring at.

For medical spas, the gap in staff utilisation between top performers and the median was 42 percentage points. It was the widest gap of any metric, in any category, in the entire report. Top performers sit around 80%. The median sits around 38%.

In plain terms: if a provider is available forty hours a week and utilised at 38%, she is delivering treatments for about fifteen of them. A top performing clinic gets around thirty two hours out of the same person.

That is not a marketing problem. Nobody needs more leads to fix that. It is the same provider, the same room, the same week.

2. Where the Gaps Actually Come From

Three sources, and they are all measurable.

Cancellations. Medical spas ran at a 14% cancellation rate in 2025, improved from 16% the year before. Roughly one in seven booked appointments does not happen as scheduled.

No shows. Medical spas have the highest no show rate of any business type in the benchmark data, at 5%.

And what the report calls calendar inflation: appointments that exist on the schedule but were never really committed to. The encouraging part is that this settles fast. Once a patient completes a second returning visit, her cancellation rate drops to about 4%. The fragility is concentrated in the early appointments. We wrote more about that pattern in our piece on patients who come once and never come back.

Add those together and a meaningful slice of your week is booked, staffed, and then quietly empty.

3. Build a Standby List. It Takes Twenty Minutes.

This is the single highest return thing in this article and most clinics have never done it.

Keep a short list of patients who have told you they would come in at short notice. That is it. No software required to start.

Where the names come from:

Anyone who asked for a time you did not have. She wanted Thursday morning, Thursday morning was full, she took the following week. She is a standby candidate and she has already told you she wants to come sooner.

Anyone who is overdue for a repeat treatment.

Anyone who cancelled and never rebooked.

Anyone who says "let me know if anything opens up," which people say constantly and clinics almost never act on.

Ask one question at checkout, every time: "If something opens up sooner, do you want me to text you?" Most people say yes. Keep the yeses in one list.

4. When a Slot Opens, Be Fast and Be Specific

The message that works is short, names the exact time, and gives a deadline. Vague offers get ignored.

"Hi Sarah, we've just had a 2pm open up today for your Botox. Want it? First to reply gets it, otherwise I'll release it at 12."

Three things are doing work there. A specific slot is a decision she can make in five seconds. The deadline creates a reason to answer now instead of later. And "first to reply" lets you message several people at once without double booking yourself.

Send it to three or four people, not one. Same day slots need volume, because most people genuinely cannot move their afternoon.

5. Fill Forward, Not Just Today

Same day gaps are the obvious version. The quieter loss is the Tuesday two weeks out that is only a third booked and nobody notices until it arrives.

Look at your calendar every Monday and find the emptiest day in the next fortnight. Then work that day deliberately. Offer it to the standby list, and to anyone overdue for a repeat treatment, before it becomes today's problem.

A gap you can see two weeks out is a scheduling task. The same gap at 11am is an emergency you will lose.

6. Turn On Online Booking, Properly

In the same benchmark data, utilisation and online booking rate showed the largest spread between top performers and the median. Those two things move together, and it is not a coincidence.

A patient deciding at 9pm that she wants to come in cannot ring you. If the only route is a phone call during business hours, you are filtering out every person whose motivation peaks outside your opening hours, which is most of them.

Two practical points. Make the booking link one tap from your Instagram profile and your Google listing, not buried on a services page. And make sure short notice slots are actually bookable. Plenty of clinics set a minimum notice window so long that today and tomorrow are invisible, which quietly guarantees the gap stays open.

7. Stop the Gap Before It Opens

Worth saying briefly, because we covered it properly in our piece on consultations: the cheapest slot to fill is the one that never empties.

A confirmation the patient has to actively respond to, and a small commitment at the point of booking, both reduce how often the hole appears in the first place. Prevention beats scramble.

8. Do This Today

Ask every patient at checkout this week whether they want a text if something opens sooner. Keep the list in one place.

Write your standby message once so nobody has to compose it under pressure.

Open your calendar and find the emptiest day in the next two weeks. Work that day now.

Check your online booking. Can someone book tomorrow? Actually try it on your phone.

None of this costs anything. All of it competes directly with your ad budget for the same outcome, which is a fuller week.

The Numbers Worth Remembering

The utilisation gap between top performing medical spas and the median was 42 percentage points, the widest gap of any metric in the 2026 benchmark data. Top performers around 80%, median around 38%.

Medical spas had a 14% cancellation rate in 2025, improved from 16%.

Medical spas have the highest no show rate of any business type in the benchmark data at 5%.

Patients who complete a second returning visit cancel at about 4%, compared to much higher rates on a first rebooking.

Utilisation and online booking rate show the widest spread between top performers and the median, and they move together.

Common Questions

What is a good utilisation rate for a med spa?
Top performers in the 2026 benchmark data sit around 80%, while the median sits around 38%. Anything in the 60s or above puts you well ahead of most clinics. The useful exercise is measuring your own and watching it move rather than chasing a number.

How do you fill a last minute cancellation?
Keep a standing list of patients who have agreed to short notice texts, then message three or four of them at once with the exact time and a deadline. Speed matters far more than the size of the list.

Should you discount a last minute slot?
Not by default. A specific time offered to someone already waiting is usually enough, and habitual discounting teaches patients to wait for gaps. Save any incentive for genuinely dead periods, and keep it small.

Why is my calendar full but my revenue flat?
Often calendar inflation. Appointments are on the books but a meaningful share cancel, particularly first rebookings. Track completed appointments rather than booked ones, because those are two different numbers.
